Is this a diagnosis or a prescription?

No. The Results Companion organizes evidence and questions around your numbers. It does not diagnose any condition and does not tell you what medication to take. Treatment decisions are made with your clinician.

Do I need to know all 7 markers to use it?

No. Most people start with a standard lipid panel (LDL-C, HDL, triglycerides). ApoB and Lp(a) are optional fields — if you don't have them yet, the Companion will note that they may be worth asking about.

Why isn't this free, like the worksheet?

The worksheet is a template. The Companion does the work of organizing your specific numbers into a specific plan — that's the part that takes real effort to build well, which is why it's a paid product.
